[1] The Moonmirror is exceptional for this fight.
Its highly telegraphed swipe attacks are perfect opportunities for Moonmirror deflections. Land one to briefly stun the boss.
The flying ice shards are fully blockable and deflectable. However, they won't deal any deflected damage to the boss.

[2] Keep running around to dodge the ice wall or shard projectiles.
The ice walls are pretty easy to avoid. When the boss burrows, keep your distance and run sideways across their projected pathing.

Ulfr Subject No. 13 is located in the Northern Tower - Sublevel Three. The platform leading to its chamber is at the far right side of the room.
Ulfr Subject No. 13 Rewards
Defeating this boss unlocks one of the four locks sealing the central door leading to the Sorcerer.
